Academic Affairs Space Request

Step 1

Complete one form for each department. Please click here to open the online Space Request Form. This form can be completed in your browser and then printed out.

Step 2

Local Room Name is the local name commonly given to the room, such as "Latin American and Caribbean Center (LACC)", "Instructional Resource Center" or "Advance Mass Spectrometry Facility". Also, include the more generic names, such as Storage Room, Reception Area, Staff Lounge, etc.

Step 3

The room use categories include: General Purpose Classroom, Training Facility, Teaching Laboratory, Research Laboratory, Computer Lab, Library, Office-Administration, Office-Faculty, Office-Graduate Assistant, Office-Clerical, Clerical/Secretarial, Conference/Meeting, Student Academic Support, Instructional Media, Media Production, Study Facilities, Animal Quarters, Central Computer or Telecommunications, Other(please specify).

Step 4

The function includes: Research, Instructional, Administrative, Other (please specify). Please make sure to note faculty and staff offices that support research.

Step 5

Enter number of occupants in each room and the number of rooms per room use. For example, number of offices, conference rooms, research labs, etc.

Step 6

Enter the net assignable square feet (NASF) per room requested and the total NASF
per room use. For example, if five offices are required at 130 NASF each, the total NASF is 650.

Step 7

Include on a separate sheet a programmatic description and justification (provide information in support of your request, including any supporting data available). Please also add a statement of the impact to the University if space is not allocated.

Step 8

Form must be submitted with the proper authorized signatures.
